Incident Investigation -- Tips, Techniques & Trivia

     On Feb 13 & 14 I'm running a two day Intermediate Level Incident Investigation program. I've had a few calls from readers asking about any prerequisites for this session and there are none.

If you've never attended one of my sessions and you are looking for a higher level of training, I suggest you come for both days. If you've taken my one day Incident Investigation & Analysis session and would like to increase you knowledge level then attend the second day of this two day session.

Writing S.M.A.R.T.E.R. Recommendations

During my training sessions I push the concept of testing the quality of your corrective actions (recommendations). Unfortunately, this is not done in the majority of investigations and organizations end up with a corrective action that does little to resolve issues over the long term.

I use the acronym SMARTER to do this. That is when you write a recommendation make it Specific, Measurable, Accountable, Reasonable, Timely, Effective and Reviewed. I'm not sure if this acronym can be attributed to any one person or organization, but I first came across it in my TapRoot training.

I've found another version of this with over thirty different terms that fit the SMARTER acronym. If you don't like the ones above you may find another term that fits your organization's culture. It doesn't matter what words you use, what matters is that you are doing something to measure the quality of your corrective actions.

Investigation Review Questionnaire

In keeping with the theme in the previous article here's a questionnaire that might help you measure your investigation and corrective action quality follow through. Examine a small sample of your 2007 reports. Are there areas of concern?
Consider the following questions:


Were the accident report forms completed at the time of the accident?
Were the accident "compensation" report forms completed at the time of the accident?
Was the CEO/GM advised of the accident when it was first reported?
Was the health and safety representative advised of the accident when it was first reported?
