I've added
a second set of public training sessions to my training calendar
this fall. Sessions will now run in both October and November,
with my new program on Serious Accident Investigation being offered
in November.

Irregularities
One of my
clients includes in their investigation reports the heading of
"Irregularities." It took me a while to warm up to this,
but now I'm a believer and am using it as a standard part of my
reporting.
So what are
these irregularities? They are simply facts or findings that because
of their unusual nature seem to jump out in front of us as we
do our investigations. In a significant investigation we are likely
to find a dozen or so of them.

Serious
Accident Investigation
(A Due Diligence Approach to the Investigation Process)
As the result
of a serious workplace accident an organization in consultation
with their legal counsel realized a better job of identifying
and collecting evidence in future accidents was needed in order
to support a due diligence defense. In response to their request
I have developed this program.
The program
is aimed at investigation teams and ideally should be attended
by all team members to ensue everyone's on the same page. The
session will be offered as a public class in November.
